The 23-year-old TOWIE star underwent a series of tests earlier this week, which have since revealed she is suffering from the long-term inflammatory bowel disease.
A spokesperson for Sam told MailOnline: 'I can confirm that Sam has very recently been diagnosed with Crohn's Disease.'
The reality star first became ill during her stint in the Celebrity Big Brother house early last month, losing a stone and a half in weight.
Crohn's Disease can lead to ulcers, abdominal pain and weight loss, and it's thought Sam - who now weighs just 7st 3lbs - will need to alter her diet dramatically to halt the progress of the condition.
The TOWIE star will also need to adhere to a liquid-only diet for six weeks to reset her bowel and will suffer relapses for the rest of her life, which could even require surgery further down the line.
A source told the Daily Mirror newspaper: 'It's Sam's diet that will be most affected by the diagnosis. She is suffering from lots of ulcers, so she needs to give them time to heal before she looks at the long term effects of diagnosis.
'At the moment, she is just trying to cope with the excruciating stomach cramps. That has been the most difficult thing. Now, she is working with her gastroenterologist to see if she can work around her illness.'
Sam fell ill while in the Celebrity Big Brother house last month and was rushed to hospital on two occasions, where she was placed on an emergency drip.
Earlier this week, Sam and her pregnant sister Billie Faiers were pictured leaving a private hospital in Chelsea after undergoing a series of blood tests, a colonoscopy, an ultrasound and a CT scan.
The former Celebrity Big Brother contestant wore a white T-shirt emblazoned with the slogan 'Choose Life' on it and a white coat as she held onto her supportive sister.
Billie – who announced on Tuesday she had got engaged to long-term boyfriend Greg Shepherd – was slightly more wrapped up in a big black jacket and a pink jumper.
Despite the wet conditions, Sam and Billie both sported sunglasses for their stroll.
But Sam looked far cheerier on Tuesday as the pair went shopping for baby accessories for Billie's impending child.
She took to her Instagram account to share a picture of her and Billie along with the caption: 'Successful baby shop today'
Billie announced her and Greg's engagement on Tuesday in a joint interview with her sister Sam, who admitted she is still not feeling '100 per cent' due to her mystery illness.
Speaking to OK! magazine, she said: 'I'm better than I was but I'm still not 100 per cent. My immune system is down at the moment so I'm picking up every bug going.
'I think I've had about nine blood tests, a CT scan and an ultrasound. They've been checking everything from my blood-sugar levels to whether I have anaemia. My bowels are swollen and so are my intestines.'
Despite not knowing what is wrong with her, doctors have reassured the blonde that her condition is 'treatable'.
She added to the publication: 'Whatever I have, the doctors have told me it's treatable and the worst-case scenario is I'd have to make some changes to my diet. It might even just be a really nasty virus.'
Last Wednesday, it was revealed Sam has shed 1st 7lb in just a month and slimmed down to a clothes size six.
A source told MailOnline at the time: 'Sam has been suffering from some strong symptoms so she went to see a doctor at the Freedom Health clinic in Harley Street on January 31st but collapsed while she was there.
'Doctors have been running blood tests on her and while she's not feeling 100%, she is now starting to feel a little better. She is hoping to get the results at the end of the week to find out what's wrong.
'Sam has lost 1st 7lb since she was on Celebrity Big Brother last month because like anyone who is ill, she has lost her appetite.
'Now she is keen to put some weight back on because she has a lot of young fans and she wants to be a good role model. She's just hoping to get to the bottom of the problem as soon as possible.'
